Soft Focus When Using The Black Foamy Thing

Neil, I like your approach when using on camera flash. I have watched many of your videos. When I use the "black foamy thing" my pictures don't seems to be as sharp. I use a Canon T1i with a 430EXII flash. My sister in-law has simular problems and she uses a Nikon D7000 with an SB700 flash. I don't think it's the camera but something I am missing.
