How can I keep all of the subject in focus and completely blur the BG with 70-200 f2.8

Hi Neil,
I saw your work on the web and it was pretty impressive...
I saw some images you took of an another female photographer with canon 200mm prime lens on a railroad track and NYC.
I have the 70-200mm 2.8 and my question to you is how were you able to get bokeh blurred out BG while keeping all of her in the frame?
What was your distance from the subject?
